Allogeneic room temperature-stored platelets (RTPs) are the standard blood product used for the management and prevention of bleeding in patients with thrombocytopenia or platelet dysfunction. These platelets are harvested from healthy donors (allogeneic) via apheresis or whole blood donation and are stored at 20-24°C with continuous agitation to maintain their viability and metabolic activity, typically for a shelf life of 5 to 7 days. In the context of the Auto-PLTS trial (NCT06115330) at the University Health Network, Toronto, RTPs serve as the standard-of-care control arm. The study evaluates the effectiveness of these standard allogeneic platelets compared to autologous cold-stored platelets in patients undergoing elective complex cardiac surgery, focusing on the reduction of allogeneic transfusion requirements and the recovery of platelet counts post-operatively.
