Overview

Allogeneic Schwann cell-derived exosomal vesicles are a novel therapeutic approach being explored for the treatment of amyotrophic lateral sclerosis (ALS) and other neurodegenerative disorders. These vesicles are extracellular organelles secreted by Schwann cells—the primary glia of the peripheral nervous system—and are harvested from healthy donor (allogeneic) sources, such as cadaveric nerve tissue. They carry a complex cargo of proteins, lipids, and RNAs that provide trophic support to motor neurons and modulate the neuroinflammatory environment. In ALS, where both motor neurons and their supporting Schwann cells are progressively compromised, the administration of these exosomes via intravenous infusion is intended to deliver regenerative signals directly to the affected tissues. Clinical evidence from expanded access cases suggests that the therapy is safe and may contribute to disease stabilization by reducing inflammasome activation and promoting axonal maintenance.
