This clinical intervention involves allogeneic hematopoietic stem cell transplantation (allo-HSCT) as a treatment for refractory systemic necrotizing vasculitis and related autoimmune disorders. Conducted at Northwestern University (NCT00278512), the protocol utilizes a high-dose immunosuppressive conditioning regimen—typically including cyclophosphamide, fludarabine, anti-thymocyte globulin (ATG), and alemtuzumab—to ablate the patient's autoreactive immune system. This is followed by the infusion of healthy donor-derived hematopoietic stem cells to reconstitute a functional, non-autoreactive immune system. The therapy is targeted at patients with severe, treatment-resistant forms of granulomatosis with polyangiitis (GPA), eosinophilic granulomatosis with polyangiitis (EGPA), and other necrotizing vasculitides.
