Allogeneic stem cells refer to hematopoietic stem cells harvested from a donor—either a 10/10 HLA-matched unrelated donor or a haploidentical donor—and infused into a recipient to treat hematologic malignancies. In the specific context of the HaploMUD trial (NCT03717116) led by Universitätsklinikum Hamburg-Eppendorf, these cells are utilized for patients with acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), or myelodysplastic syndromes (MDS) who are in complete remission. The therapeutic mechanism involves the reconstitution of the recipient's hematopoietic and immune systems, followed by the induction of a graft-versus-leukemia (GVL) effect. In this effect, donor-derived T-cells and natural killer cells recognize and eliminate residual malignant cells, thereby reducing the risk of relapse. The HaploMUD study is a randomized Phase 2 trial comparing the anti-leukemic activity and safety of haploidentical versus matched unrelated donor transplants, both utilizing post-transplant cyclophosphamide (PTCy) as the primary graft-versus-host disease (GVHD) prophylaxis.
