Allogeneic umbilical cord-derived mesenchymal stem cells (UC-MSCs) are an investigational cell therapy being evaluated by Korea University Anam Hospital for the treatment of large to massive rotator cuff tears. These cells are harvested from the umbilical cord, a rich source of primitive mesenchymal stem cells with high proliferative capacity and low immunogenicity. In the clinical setting, the cells are administered via a single ultrasound-guided injection directly into the lesion site, often as an adjunct to arthroscopic rotator cuff repair. The therapeutic mechanism involves the secretion of paracrine factors, including growth factors and cytokines, which modulate the inflammatory environment and promote the regeneration of the tendon-to-bone interface (enthesis). This approach aims to enhance the biological healing process and reduce the high rate of re-tears associated with surgical repair of massive rotator cuff injuries.
