Allogeneic umbilical cord mesenchymal stromal cells (UC-MSCs) are multipotent progenitor cells harvested from the Wharton's jelly of human umbilical cords. These cells are characterized by their low immunogenicity and significant immunomodulatory potential, which they exert through the secretion of various cytokines, chemokines, and extracellular vesicles, as well as through direct cell-to-cell contact. In the treatment of type 1 diabetes mellitus, UC-MSCs aim to arrest the autoimmune destruction of insulin-producing pancreatic beta cells by shifting the immune response from a pro-inflammatory Th1/Th17 profile toward a regulatory T-cell (Treg) dominated environment. The Affiliated Nanjing Drum Tower Hospital is evaluating these cells in clinical trials to determine if repeated infusions can safely preserve residual C-peptide secretion and improve glycemic control in patients with recently diagnosed or established type 1 diabetes.
