Allogeneic umbilical cord tissue-derived mesenchymal stromal cells (UC-MSCs) are an ex vivo expanded cell therapy developed by Assistance Publique - Hôpitaux de Paris (AP-HP). These cells are isolated from the Wharton's jelly of the umbilical cord and expanded under Good Manufacturing Practice (GMP) conditions. UC-MSCs possess potent immunomodulatory and anti-inflammatory properties, making them a promising candidate for treating chronic active antibody-mediated rejection (caABMR) in kidney transplant recipients. Their mechanism of action involves the secretion of paracrine factors such as indoleamine 2,3-dioxygenase (IDO), prostaglandin E2 (PGE2), and various cytokines that suppress the activation and proliferation of T-lymphocytes, B-lymphocytes, and natural killer (NK) cells. Additionally, they promote the induction and expansion of regulatory T-cells (Tregs), thereby shifting the immune balance toward graft tolerance and potentially reducing the levels of donor-specific antibodies (DSAs) that drive chronic rejection.
