Overview

alloMILs (activated donor-derived marrow infiltrating lymphocytes) is an adoptive cellular therapy developed by researchers at Johns Hopkins University for the treatment of hematologic malignancies relapsing after allogeneic hematopoietic cell transplantation (HCT). Unlike traditional donor lymphocyte infusions (DLI) which use peripheral blood T cells, alloMILs are derived from the donor's bone marrow, which is naturally enriched for tumor-specific memory T cells. The production process involves harvesting bone marrow mononuclear cells from the original transplant donor, followed by ex vivo activation and expansion using anti-CD3/CD28-coated beads and interleukin-2 (IL-2). This approach aims to restore graft-versus-tumor (GVT) effects while minimizing the risk of graft-versus-host disease (GVHD), particularly in patients who received post-transplantation cyclophosphamide (PTCy) as prophylaxis.
