Alloreactive NK cells (IRCCS Azienda Ospedaliero-Universitaria di Bologna) is an adoptive cell therapy being developed for the treatment of acute myeloid leukemia (AML). The therapy utilizes highly purified CD56+CD3- natural killer (NK) cells harvested from haploidentical donors who exhibit a KIR-ligand mismatch with the recipient. This mismatch prevents the inhibition of NK cells by the recipient's self-HLA molecules, thereby triggering potent graft-versus-leukemia (GvL) activity. The cells are administered as a consolidation strategy for adult AML patients in complete remission who are not candidates for allogeneic stem cell transplantation. The treatment protocol includes lymphodepleting chemotherapy with fludarabine and cyclophosphamide followed by the NK cell infusion and subsequent interleukin-2 (IL-2) administration to support cell expansion and persistence.
