Overview

AlloStim + avelumab is a combination immunotherapy being studied in patients with refractory metastatic colorectal cancer. AlloStim is an experimental allogeneic, non-genetically manipulated, Th1-like activated living cell immunotherapy derived from healthy unrelated blood donors, developed to convert immunologically "cold" tumors into "hot" tumors infiltrated by interferon-gamma producing Th1 cells. This priming is hypothesized to increase PD-L1 expression in the tumor microenvironment, thereby potentially enhancing response to anti-PD-L1 checkpoint inhibition. Avelumab is a human monoclonal antibody that acts as a checkpoint inhibitor by binding to programmed death ligand-1 (PD-L1), blocking its interaction with PD-1 to promote immune-mediated tumor cell killing. The combination is being investigated specifically as a fourth-line (4L) therapy for patients with microsatellite stable (MSS)/proficient mismatch repair (pMMR) metastatic colorectal cancer, a population generally unresponsive to checkpoint inhibitors alone. The current phase 2 trial involves a sequence of AlloStim priming followed by combined administration of AlloStim and avelumab[1][3][4][5][7][8].
