Overview

Alpha-keto-isoleucine is a keto acid analog of the essential amino acid isoleucine, chemically known as calcium 3-methyl-2-oxopentanoate. It is used primarily as a nutritional supplement in the management of chronic kidney disease (CKD), particularly to help delay progression and reduce nitrogenous waste in patients on low-protein diets. Alpha-keto-isoleucine follows similar catabolic pathways as amino acids and works by improving protein metabolism, thereby supporting renal function. It is often included in formulations referred to as "alpha ketoanalogues," which are mixtures of several keto acid analogs of essential amino acids used for supportive care in CKD[1][3][5].
