Overview

Alpha tocopherol acetate (also known as vitamin E acetate) is a synthetic or natural ester form of vitamin E (alpha-tocopherol). It is a fat-soluble small molecule drug and dietary supplement widely used to treat and prevent vitamin E deficiency, including ataxia with vitamin E deficiency (AVED). Upon oral administration, alpha tocopherol acetate is hydrolyzed in the intestine to release active alpha-tocopherol, which functions primarily as a potent antioxidant. It integrates into cellular membranes and scavenges peroxyl free radicals, thereby protecting membrane lipids from oxidative damage and lipid peroxidation. Beyond its antioxidant properties, alpha-tocopherol acts as a cell signaling modulator by directly binding to and inhibiting protein kinase C alpha (PKC alpha), which contributes to its anti-inflammatory, antiproliferative, and immune-enhancing effects. It also binds to the hepatic alpha-tocopherol transfer protein (alpha-TTP), facilitating its secretion and distribution into circulating lipoproteins. Alpha tocopherol acetate has been extensively studied in Phase 3 clinical trials for various conditions associated with oxidative stress, including Alzheimer's disease, non-alcoholic steatohepatitis (NASH), tardive dyskinesia, cardiovascular disease, cataracts, and the prevention of cisplatin-induced neuropathy.
