Overview

Alpha-tocopheryloxyacetic acid + trastuzumab is an investigational combination therapy for HER2-positive breast cancer. Alpha-tocopheryloxyacetic acid (α-TEA) is a semi-synthetic ether derivative of vitamin E (alpha-tocopherol) with selective cytotoxicity against cancer cells. Trastuzumab is a monoclonal antibody targeting the HER2/neu receptor, commonly overexpressed in aggressive breast cancers. Preclinical studies have shown that α-TEA induces apoptosis in both HER2-positive and negative breast cancer cells and enhances anti-tumor immune responses by increasing activated effector memory CD4+ and CD8+ T cells while reducing regulatory T cell populations. When combined with trastuzumab, α-TEA has demonstrated synergistic effects leading to faster tumor regression and higher rates of complete response compared to either agent alone[1][2][4]. The combination aims to provide effective anti-cancer activity with potentially less toxicity than conventional chemotherapies. Currently, this combination is being evaluated in phase 1 clinical trials for patients with refractory or metastatic HER2-positive breast cancer[3][8][9]. The primary goals are to assess safety, determine optimal dosing of α-TEA when used alongside standard-dose trastuzumab, and evaluate preliminary efficacy as well as immunomodulatory effects.
