alphalex exatecan (CBX-12) is a clinical-stage, first-in-class peptide drug conjugate that uses Cybrexa’s proprietary alphalex tumor-targeting platform to deliver the highly potent topoisomerase I inhibitor exatecan selectively into tumor cells via a pH-Low Insertion Peptide (pHLIP), a cleavable linker, and an exatecan payload.[2][3][7][13] By exploiting the acidic microenvironment of solid tumors to drive pH-dependent peptide insertion into cancer cell membranes and intracellular release of exatecan, CBX-12 aims to improve the therapeutic index of topoisomerase I inhibition, enabling higher effective dosing with reduced systemic toxicity and without dependence on target antigen expression, and is being developed primarily for advanced solid tumors including platinum-resistant ovarian cancer, hormone receptor–positive/HER2-negative breast cancer, colorectal cancer, and other refractory solid malignancies.[2][3][5][7][8][10][11][16][17]
