Overview

Alphamethadol is a synthetic opioid analgesic and an isomer of dimepheptanol (methadol), with the other being betamethadol. It consists of two stereoisomers, L-α-methadol and D-α-methadol. Both isomers bind to and activate the μ-opioid receptor, producing opioid analgesic effects similar to those of methadone and alphacetylmethadol. Alphamethadol has a high potential for abuse, lacks accepted medical use in the United States, and is classified as a Schedule I controlled substance by the DEA[1][2][3]. L-α-methadol serves as an active metabolite of levacetylmethadol (LAAM), which was previously used in opioid addiction therapy[2][3].
