ALPP CAR-T cells are autologous chimeric antigen receptor (CAR) T cell therapies engineered to target alkaline phosphatase, placental (ALPP), a cell surface protein overexpressed in certain solid tumors such as ovarian, endometrial, and colorectal cancers. The therapy involves collecting a patient's own T lymphocytes, genetically modifying them ex vivo using a retroviral vector to express an anti-ALPP CAR construct, and then reinfusing the modified T cells back into the patient. Upon encountering ALPP-expressing tumor cells, these engineered T cells become activated and mediate targeted cytotoxicity against the cancer. Preclinical studies have demonstrated that ALPP is largely restricted to placental and testicular tissues among normal adult tissues but is aberrantly expressed in several malignancies. Early clinical trials are evaluating safety and efficacy in patients with advanced solid tumors who have failed standard therapies[1][2][4][6][10].
