Overview

Alstonia scholaris leaf alkaloids (CALAS) are a botanical drug extract derived from the leaves of *Alstonia scholaris* (L.) R.Br., a plant in the Apocynaceae family. This investigational drug is primarily developed for respiratory conditions such as bronchitis, post-infectious cough, and asthma. The active components include several indole alkaloids such as picrinine, vallesamine, and scholaricine which have demonstrated anti-inflammatory effects by inhibiting inflammatory mediators including cyclooxygenase-1 (COX-1), cyclooxygenase-2 (COX-2), and 5-lipoxygenase (5-LOX). These alkaloids also enhance antioxidant enzyme activity like superoxide dismutase while reducing nitric oxide, prostaglandin E2, and malondialdehyde levels. Clinical studies indicate antitussive activity by suppressing cough frequency in animal models. A phase I clinical trial showed CALAS to be safe and well-tolerated up to doses of 360 mg single dose or 120 mg three times daily for seven days in healthy volunteers with only mild transient adverse events reported.
