Overview

ALT-801 + gemcitabine is a combination therapy that merges **ALT-801**, a bifunctional fusion protein comprising **interleukin-2 (IL-2) linked to a single-chain T cell receptor domain specific for the p53 (aa264-272) peptide presented by HLA-A*0201 on tumor cells**, with **gemcitabine**, a nucleoside analog chemotherapeutic agent. ALT-801 targets and activates immune effector cells at the tumor microenvironment through IL-2 receptor agonism and tumor-associated antigen specificity, leading to immune cell infiltration and antitumor activity. Gemcitabine works by inhibiting DNA synthesis in rapidly dividing tumor cells, and also can increase p53 expression on the tumor surface, thereby potentially enhancing ALT-801 targeting. The combination has been primarily investigated for **muscle invasive or metastatic urothelial cancer** through clinical trials with aims to assess safety, maximum tolerated dose, recommended dose, pharmacokinetic profile, and antitumor response[1][3][4][5].
