Overview

The combination of **alteplase** and **argatroban** is a therapeutic regimen being evaluated for the treatment of acute posterior circulation infarction. Alteplase is a recombinant tissue plasminogen activator (rt-PA) that functions as a thrombolytic agent by converting plasminogen to plasmin, which subsequently degrades fibrin clots. Argatroban is a small-molecule direct thrombin inhibitor that reversibly binds to the thrombin active site, preventing fibrin formation and platelet aggregation. This combination is the focus of the Phase 4 **AR-PCI** (Argatroban Plus r-tPA for Posterior Circulation Infarction) trial led by researcher **Hui-Sheng Chen**. The study aims to determine if adding argatroban to standard alteplase therapy can improve clinical outcomes and vessel recanalization rates in patients with ischemic strokes involving the posterior circulation, without significantly increasing the risk of hemorrhage.
