Alteplase + tirofiban is a combination therapy investigated for the treatment of acute ischemic stroke (AIS). Alteplase is a recombinant tissue plasminogen activator (rt-PA) that promotes fibrinolysis by converting plasminogen to plasmin, which subsequently dissolves fibrin-rich clots. Tirofiban is a small-molecule, non-peptide antagonist of the platelet glycoprotein (GP) IIb/IIIa receptor, which inhibits platelet aggregation by preventing the binding of fibrinogen and von Willebrand factor to platelets. The combination is designed to improve recanalization rates and prevent early re-occlusion in stroke patients by addressing both the fibrin and platelet components of the thrombus. The Department of Neurology at the Affiliated Yantai Yuhuangding Hospital of Qingdao University has been a key developer in evaluating this combination, particularly focusing on its safety and efficacy in patients with acute ischemic stroke who may benefit from enhanced antiplatelet and thrombolytic action, such as those with large vessel occlusion or those undergoing endovascular treatment.
