It exerts its therapeutic effect by inhibiting the sodium-chloride symporter (SLC12A3) in the distal convoluted tubule of the nephron, which increases the renal excretion of sodium, chloride, and water, thereby reducing extracellular fluid volume and blood pressure. While it can be used as a monotherapy, altizide is most commonly encountered in fixed-dose combinations with the potassium-sparing diuretic spironolactone (notably under the brand name Aldactazine) to mitigate the risk of hypokalemia.
