Overview

ALTO-101 is a novel small molecule phosphodiesterase-4 (PDE4) inhibitor being developed by Alto Neuroscience for the treatment of cognitive impairment associated with schizophrenia (CIAS), a condition characterized by negative and cognitive symptoms for which there are currently no targeted treatments. The drug works by inhibiting PDE4, thereby increasing cyclic adenosine monophosphate (cAMP) levels, which is believed to have pro-cognitive effects in the central nervous system. ALTO-101 has been formulated both as an oral medication and as a proprietary transdermal patch developed in partnership with MEDRx. The transdermal formulation provides greater systemic drug exposure and fewer adverse events typically associated with PDE4 inhibitors compared to oral administration. In Phase 1 clinical trials, ALTO-101 demonstrated human brain penetration, robust CNS-relevant pharmacodynamic effects, favorable tolerability, and improved pharmacokinetics. It is currently in Phase 2 clinical development for CIAS[1][2][3][5].
