ALV001 is a recombinant, modified form of the cysteine endoprotease B2 (EPB2) zymogen derived from barley (*Hordeum vulgare*). It is a biologic enzyme designed to degrade gluten proteins in the stomach before they reach the small intestine. ALV001 is specifically activated at acidic pH (below 5.0), which allows it to function in the gastric environment. It targets the glutamine-rich and proline-rich sequences of gluten that are resistant to human digestive enzymes and trigger the autoimmune response in celiac disease. ALV001 was developed by Alvine Pharmaceuticals and is most commonly studied as a component of the combination therapy ALV003 (latiglutenase), where it works synergistically with a prolyl endopeptidase (PEP) to achieve more complete gluten detoxification.
