**Alvocidib + azacitidine** is an experimental combination drug consisting of the cyclin-dependent kinase inhibitor alvocidib (also known as flavopiridol) and the hypomethylating agent azacitidine (also called 5-azacytidine). Alvocidib acts mainly as a CDK9 inhibitor, indirectly repressing the anti-apoptotic protein MCL-1 and inducing apoptosis in hematologic malignancies. Azacitidine, a nucleoside analog, inhibits DNA methyltransferases, resulting in hypomethylation of DNA and reactivation of silenced genes, which leads to cell differentiation or apoptosis in malignant cells. Preclinical and early clinical data suggest these agents exert additive or synergistic cytotoxic effects in high-risk myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML), and can overcome resistance mechanisms, including those linked to MCL-1 and ASXL1 mutation status[2][3][4]. The combination has been investigated in high-risk MDS and AML, with particular sensitivity shown in the ASXL1-mutated subgroup.
