Overview

Alvocidib + cytarabine + daunorubicin is a combination regimen consisting of three distinct drugs: alvocidib (a cyclin-dependent kinase inhibitor), cytarabine (a nucleoside analog and anti-metabolite), and daunorubicin (an anthracycline topoisomerase II inhibitor). This combination is primarily researched for the treatment of acute myeloid leukemia (AML), especially in high-risk or therapy-related subtypes, by combining mechanisms that disrupt cell cycle control (alvocidib), DNA synthesis (cytarabine), and induce DNA damage/apoptosis (daunorubicin). This regimen is designed to enhance the antileukemic efficacy by targeting multiple pathways in leukemic blasts. Alvocidib inhibits cyclin-dependent kinases (especially CDK9), leading to downregulation of anti-apoptotic proteins (like MCL-1), promoting apoptosis when paired with cytarabine and daunorubicin which further impede DNA replication and repair.
