ALX-009 is an investigational antimicrobial therapy composed of two naturally occurring components of the immune system: hypothiocyanite ion (OSCN-) and bovine lactoferrin (bLF). These substances are normally present in the airway liquid of healthy individuals as part of the innate immune system but are deficient in cystic fibrosis (CF) patients. The combination works by restoring the natural capacity of the lungs to fight infections, targeting multiple bacterial mechanisms which makes it particularly effective against multi-drug resistant gram-negative bacteria. ALX-009 has demonstrated bactericidal activity against *Pseudomonas aeruginosa* and *Burkholderia cepacia* complex, key pathogens in CF lung infections, even in complex biological matrices like biofilms and sputum where traditional antibiotics often fail. The drug is administered via inhalation using a dedicated equipment called EOLEASE that produces the drug on-site just before administration. It is being developed by Alaxia SAS.
