ALX-0141 is a bivalent Nanobody (single-domain antibody) that specifically targets and inhibits the Receptor Activator of Nuclear Factor kappa B Ligand (RANKL), a key mediator in osteoclast differentiation and bone resorption. By blocking RANKL, ALX-0141 prevents its interaction with the RANK receptor on osteoclasts, thereby inhibiting osteoclast formation and activity. This mechanism is intended to reduce bone loss in conditions characterized by excessive bone resorption. The drug was developed by Ablynx for potential use in degenerative bone diseases such as postmenopausal osteoporosis and cancer-related bone disorders. Phase I clinical trials demonstrated that ALX-0141 was well tolerated over a wide range of doses with no serious adverse events or dose-limiting toxicity observed. It showed favorable pharmacokinetics and sustained suppression of the bone resorption biomarker CTX-1 for up to 390 days at higher doses[3][5][7][8].
