Ambenonium is a small molecule cholinesterase inhibitor used in the management of myasthenia gravis. It acts as a competitive and reversible inhibitor of acetylcholinesterase, the enzyme responsible for hydrolyzing acetylcholine. By inhibiting this enzyme, ambenonium increases acetylcholine levels at neuromuscular junctions, thereby enhancing cholinergic transmission and improving muscle strength in patients with myasthenia gravis. Ambenonium was marketed under the brand name Mytelase but has been discontinued in the United States since 2012[1][3][5][7]. It produces fewer muscarinic side effects than neostigmine but more than pyridostigmine[1][2]. The drug is administered orally and has low bioavailability due to poor gastrointestinal absorption[2].
