American ginseng (Panax quinquefolius) is a botanical dietary supplement that was evaluated by Johns Hopkins University for the treatment of HIV-associated fatigue. The intervention used in clinical studies was a standardized whole-root powder encapsulated in 500 mg doses, containing at least 5% total ginsenosides. Ginsenosides are saponin-class plant steroids believed to function as adaptogens. The proposed mechanism involves the modulation of systemic inflammation and the reduction of proinflammatory cytokines, which are associated with chronic fatigue in HIV-infected patients on antiretroviral therapy. A Phase 2 trial (NCT01500096) investigated the efficacy and safety of 1000 mg and 3000 mg daily oral doses.
