AMG 386 + cisplatin + capecitabine is a combination regimen evaluated in clinical trials primarily for metastatic gastro-oesophageal cancer and other advanced solid tumors. AMG 386 (also known as trebananib) is a recombinant peptibody that acts as an antiangiogenic agent by neutralizing angiopoietin-1 and -2, thereby preventing their interaction with the Tie2 receptor on endothelial cells and disrupting tumor angiogenesis. Cisplatin is a platinum-based chemotherapeutic drug that induces DNA crosslinking, leading to apoptosis in rapidly dividing cells. Capecitabine is an orally administered prodrug of 5-fluorouracil, a pyrimidine analog interfering with DNA synthesis. The combination is designed to leverage both direct cytotoxic effects (from cisplatin and capecitabine) and antiangiogenic mechanisms (from AMG 386) to target tumor growth and vascular support.
