This is a combination chemotherapy regimen consisting of three agents—amifostine, cisplatin, and etoposide. Amifostine is a cytoprotective agent that acts as a prodrug, converted in tissues to an active thiol metabolite which scavenges free radicals and protects normal tissues from the toxic effects of chemotherapy and radiation. Cisplatin is a platinum-based chemotherapeutic that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. Etoposide is a topoisomerase II inhibitor that induces DNA strand breaks by preventing the religation of double-stranded DNA during replication. This combination has been studied primarily for its potential to reduce the toxicity (notably nephrotoxicity and ototoxicity) associated with cisplatin-based regimens while maintaining antitumor efficacy, particularly in pediatric germ-cell tumors and small cell lung cancer[1][4][7]. Amifostine's ability to protect against certain toxicities (e.g., nephrotoxicity) has been demonstrated; however, its effectiveness at reducing ototoxicity when combined with high-dose cisplatin plus etoposide remains inconclusive[1][3]. The regimen may be used with or without additional agents such as bleomycin or concurrent radiotherapy depending on indication.
