This is a combination of three antibiotics—amikacin, linezolid, and vancomycin—used together for the treatment of severe or multidrug-resistant bacterial infections. Each component has a distinct mechanism of action: - **Amikacin** is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit, inhibiting protein synthesis and causing misreading of mRNA in bacteria. - **Linezolid** is an oxazolidinone antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, preventing formation of the initiation complex. - **Vancomycin** is a glycopeptide antibiotic that inhibits cell wall biosynthesis by binding to D-Ala-D-Ala termini in peptidoglycan precursors. The combination may be considered for difficult-to-treat infections caused by resistant Gram-positive organisms (such as MRSA or VRE) or mixed infections where broad-spectrum coverage is needed. Synergistic effects have been observed between some pairs (e.g., amikacin with vancomycin[2], linezolid with aminoglycosides[1]), but antagonism has also been reported between vancomycin and linezolid in certain strains[5][7]. The use of all three drugs together would be reserved for highly resistant cases under specialist supervision.
