Overview

Amikacin + meropenem is a combination of two antibiotics—amikacin, an aminoglycoside, and meropenem, a carbapenem β-lactam—used together primarily for the empirical treatment of serious infections caused by Gram-negative bacteria, including multidrug-resistant strains such as Pseudomonas aeruginosa. The combination leverages the distinct mechanisms of action of each drug to achieve synergistic antibacterial effects and reduce the risk of resistance development. Amikacin inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, while meropenem inhibits cell wall synthesis by binding to penicillin-binding proteins. This dual approach disrupts multiple essential bacterial processes simultaneously. Combination therapy is particularly considered in critically ill or immunocompromised patients and in cases where resistance to one agent (e.g., carbapenems) is suspected or confirmed[1][2][4]. Metabolomic studies show that this combination rapidly perturbs key metabolic pathways in bacteria—including those involved in cell wall/membrane synthesis and central carbon metabolism—leading to enhanced bactericidal activity compared with monotherapy[4]. Clinical guidelines recommend initial use for severe infections pending susceptibility results.
