Amikacin + nafcillin is a combination of two antibiotics used for the treatment of severe bacterial infections, particularly those caused by both Gram-negative and Gram-positive organisms. Amikacin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to cell death. It is especially effective against resistant Gram-negative bacteria such as Pseudomonas aeruginosa and Acinetobacter species. Nafcillin is a beta-lactamase-resistant penicillin (antistaphylococcal penicillin) that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins; it is primarily used for infections caused by methicillin-sensitive Staphylococcus aureus (MSSA) and other susceptible staphylococci[3][5][6]. The combination may be considered in clinical scenarios where mixed or complicated infections are suspected or confirmed, such as endocarditis or bacteremia involving both susceptible staphylococci and resistant Gram-negative pathogens.
