Overview

Arikayce (amikacin liposome) is an inhaled aminoglycoside antibiotic formulated as a liposomal suspension for oral inhalation. It is specifically indicated for the treatment of Mycobacterium avium complex (MAC) lung disease in adults who have limited or no alternative treatment options and who do not achieve negative sputum cultures after at least six consecutive months of a multidrug background regimen. The drug works by delivering both free and encapsulated amikacin directly to the lungs via nebulization, allowing higher concentrations to reach macrophages and biofilms where MAC bacteria persist. Amikacin acts by binding to bacterial 30S ribosomal subunits, inhibiting protein synthesis and leading to bacterial cell death. Arikayce was developed by Insmed and is approved under accelerated approval based on sputum culture conversion; its clinical benefit has not yet been fully established[1][4][6].
