Overview

Amiloride is a potassium-sparing diuretic used primarily in combination with other diuretics to treat hypertension (high blood pressure), congestive heart failure, and edema, especially in patients at risk of low potassium levels. It acts by directly blocking the epithelial sodium channel (ENaC) in the distal convoluted tubule, connecting tubule, and collecting duct of the nephron. This inhibition reduces sodium reabsorption and promotes excretion of sodium and water while minimizing potassium loss. Amiloride is not an aldosterone antagonist; its effects are independent of aldosterone presence. The drug may also inhibit Na+/H+ exchangers such as NHE-1 on cardiac tissue and has some action on acid-sensing ion channels (ASICs). It is administered orally as a tablet[1][2][5][6][7].
