Aminoglycoside + first-generation cephalosporin is an antibiotic combination regimen being evaluated by the Methodist Health System for the prevention of surgical site infections in patients with Gustilo Type II or III open lower extremity fractures. This combination therapy pairs an aminoglycoside (such as gentamicin or tobramycin), which inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, with a first-generation cephalosporin (such as cefazolin), which disrupts bacterial cell wall synthesis by binding to penicillin-binding proteins. The regimen is currently being studied in a Phase 3 clinical trial (NCT05267977) to determine if the dual-coverage approach provides superior clinical outcomes compared to cephalosporin monotherapy. This investigation follows retrospective data from Methodist Dallas Medical Center suggesting that the addition of aminoglycosides may increase the risk of acute kidney injury without significantly reducing infection rates in this specific trauma population.
