Aminoleban is a specialized amino acid solution designed for the treatment of hepatic encephalopathy and nutritional support in patients with chronic liver disease. Developed by Otsuka Pharmaceutical, it is formulated with a high concentration of branched-chain amino acids (BCAA)—leucine, isoleucine, and valine—and a low concentration of aromatic amino acids (AAA) and methionine. This specific ratio, often referred to as the Fischer ratio, is intended to correct the plasma amino acid imbalance characteristic of liver failure. By increasing BCAA levels, Aminoleban competes with AAAs for transport across the blood-brain barrier via the LAT1 transporter, thereby reducing the synthesis of false neurotransmitters that contribute to encephalopathy. Additionally, it promotes protein synthesis and helps suppress muscle protein breakdown, improving the overall metabolic state of patients with decompensated liver cirrhosis.
