Overview

Aminophylline + dexmedetomidine is a combination of two pharmaceutical agents used in perioperative and anesthetic settings. Aminophylline is a bronchodilator and central nervous system stimulant, primarily acting as a nonselective phosphodiesterase inhibitor and adenosine receptor antagonist, leading to increased intracellular cAMP, bronchodilation, cardiac stimulation, and reduced sedation. Dexmedetomidine is a highly selective α2-adrenoceptor agonist that provides sedation, analgesia, anxiolysis, and sympatholytic effects by inhibiting neuronal firing in the brain and spinal cord. This combination has been studied for its ability to counteract the sedative effects of dexmedetomidine during procedures requiring controlled hypotension (such as functional endoscopic sinus surgery), with aminophylline shown to enhance recovery profiles by reducing extubation time and postoperative sedation through antagonism of adenosine receptors mediating hypnosis[2][3][5].
