Overview

Aminophylline + hydrocortisone is a combination of two pharmaceutical agents used primarily in the management of acute asthma exacerbations and other respiratory conditions characterized by bronchospasm and inflammation. Aminophylline is a bronchodilator, classified as a methylxanthine, which acts mainly by inhibiting phosphodiesterase enzymes (leading to increased cyclic AMP), blocking adenosine receptors, and activating histone deacetylase. These actions result in relaxation of bronchial smooth muscle, increased diaphragmatic contractility, mild anti-inflammatory effects, and central nervous system stimulation[2][7][9]. Hydrocortisone is a corticosteroid that exerts potent anti-inflammatory and immunosuppressive effects by modulating gene expression through the glucocorticoid receptor. It reduces airway inflammation, decreases leukotriene production (notably leukotriene C4), suppresses immune cell activity, and stabilizes cellular membranes[4][5][8]. The combination may be used intravenously in acute settings to provide both rapid bronchodilation (aminophylline) and reduction of airway inflammation (hydrocortisone). This dual approach can be beneficial for patients with severe asthma attacks or status asthmaticus who do not respond adequately to inhaled therapies.
