Aminophylline + magnesium sulphate is a combination of two small molecule drugs, each with distinct but potentially complementary mechanisms of action. Aminophylline is a bronchodilator composed of theophylline and ethylenediamine, primarily used to treat bronchospasm in conditions such as asthma and chronic obstructive pulmonary disease (COPD). It acts mainly by inhibiting phosphodiesterase enzymes, antagonizing adenosine receptors, and activating histone deacetylase, resulting in smooth muscle relaxation in the airways and anti-inflammatory effects[4][5][8]. Magnesium sulphate is an electrolyte replenisher with anticonvulsant properties that also exhibits smooth muscle relaxant effects. In acute asthma management, it works by inhibiting calcium influx into airway smooth muscle cells, reducing acetylcholine and histamine release from nerve terminals and mast cells respectively, promoting nitric oxide synthesis for broncho- and vasodilation, and attenuating neutrophil activation[6][7]. The combination has been studied for adjunctive use in severe acute asthma unresponsive to first-line therapies as well as for maternal administration to reduce intraventricular hemorrhage risk in preterm neonates[2]. Clinical evidence suggests potential benefit when both agents are administered together under specific circumstances.
