The therapy involves the administration of either amiodarone, sotalol, or mexiletine, depending on the patient's prior treatment history and the clinician's discretion. Amiodarone is a Class III antiarrhythmic that blocks potassium, sodium, and calcium channels and has alpha- and beta-adrenergic blocking properties. Sotalol is a non-selective beta-blocker with Class III antiarrhythmic properties (potassium channel blockade). Mexiletine is a Class Ib antiarrhythmic that blocks inward sodium channels. This pharmacological approach is being compared against repeat catheter ablation guided by non-invasive programmed stimulation (NIPS) to reduce VT recurrence in patients with structural heart disease.
