Amiprilose is a synthetic carbohydrate derivative, specifically a substituted D-glucofuranose, that possesses immunomodulatory and anti-inflammatory properties. Developed primarily for the treatment of rheumatoid arthritis, it functions by modulating the production of pro-inflammatory cytokines such as interleukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-alpha), and by inhibiting the proliferation of synovial fibroblasts. Although it demonstrated a favorable safety profile and some efficacy in early clinical trials as a disease-modifying antirheumatic drug (DMARD), its development was ultimately discontinued after failing to meet primary endpoints in pivotal Phase 3 studies and receiving a non-approvable letter from the FDA in the early 1990s.
