amisulpride + mirtazapine

Amisulpride + mirtazapine is a combination of two pharmaceutical drugs used off-label, primarily in the management of psychotic depression and potentially other psychiatric conditions. Amisulpride is an atypical antipsychotic that acts as a selective dopamine D2 and D3 receptor antagonist, with preferential activity in the limbic system, leading to antipsychotic effects with relatively fewer extrapyramidal side effects. Mirtazapine is an antidepressant classified as a noradrenergic and specific serotonergic antidepressant (NaSSA). It works mainly by antagonizing central presynaptic alpha-2 adrenergic inhibitory autoreceptors and heteroreceptors, increasing norepinephrine and serotonin release; it also blocks serotonin 5-HT2 and 5-HT3 receptors as well as histamine H1 receptors, contributing to its sedative properties. The combination has been studied for efficacy in elderly patients with psychotic depression, showing potential benefit for both depressive and psychotic symptoms[1][4]. However, coadministration may increase the risk of QT interval prolongation and ventricular arrhythmias[7].
