This is a combination of three tricyclic antidepressants (TCAs): amitriptyline, clomipramine, and imipramine. All three are small molecule drugs that act primarily by inhibiting the reuptake of serotonin and norepinephrine in the central nervous system, thereby increasing their synaptic concentrations. Amitriptyline is indicated for depressive illness and chronic pain syndromes; it has strong anticholinergic and sedative properties[3][4]. Clomipramine is most notable for its use in obsessive-compulsive disorder but also treats depression, panic disorder, chronic pain (especially tension-type headache), cataplexy associated with narcolepsy, and other conditions[5]. Imipramine is used for depression as well as childhood enuresis. The combination of these agents would theoretically provide robust inhibition of serotonin and norepinephrine reuptake due to their overlapping but distinct pharmacological profiles. However, combining multiple TCAs significantly increases the risk of additive side effects such as anticholinergic toxicity (dry mouth, blurred vision), sedation, cardiovascular effects (arrhythmias), CNS depression, seizures, and potential drug interactions[2][9]. There are no widely recognized brand names or clinical guidelines supporting the routine use of this specific triple TCA combination.
