Amitriptyline + clozapine is a combination of two small molecule drugs, amitriptyline and clozapine. Amitriptyline is a tricyclic antidepressant with anticholinergic and sedative properties, primarily used for depression, neuropathic pain, and off-label for conditions such as fibromyalgia. Its mechanism involves inhibition of serotonin and norepinephrine reuptake transporters in the central nervous system. Clozapine is an atypical antipsychotic indicated for treatment-resistant schizophrenia; it acts mainly by antagonizing dopamine D2 receptors and serotonin 5-HT2A receptors, among others. The combination may be considered in rare clinical scenarios (e.g., using low-dose amitriptyline to manage clozapine-induced sialorrhea), but carries significant risk due to major drug-drug interactions that can lead to toxic serum levels or severe adverse effects such as pulmonary toxicity[1][2][4].
