This is a combination of three pharmaceutical drugs: - **Amitriptyline** is a tricyclic antidepressant (TCA) that inhibits the reuptake of serotonin and norepinephrine, increasing their levels in the central nervous system. It is primarily used to treat depression and certain types of chronic pain. - **Fluoxetine** is a selective serotonin reuptake inhibitor (SSRI) that increases synaptic serotonin by inhibiting its reuptake. It is commonly prescribed for depression, anxiety disorders, and other psychiatric conditions. - **Tramadol** is an opioid analgesic with additional activity as a weak inhibitor of serotonin and norepinephrine reuptake. It provides pain relief through mu-opioid receptor agonism as well as monoaminergic effects. The combination of these three agents significantly increases serotonergic activity in the central nervous system, raising the risk for serious adverse effects such as serotonin syndrome and seizures. Tramadol’s metabolism can be inhibited by fluoxetine (a CYP2D6 inhibitor), leading to higher tramadol concentrations and further increasing serotonergic toxicity risk[1][4][5][6]. Combining these medications should generally be avoided unless there are compelling clinical reasons under close medical supervision.
