Caramlo HCT is a fixed-dose combination small molecule drug developed by Apontis Pharma for the treatment of essential hypertension in adults. It contains three active pharmaceutical ingredients: candesartan cilexetil (an angiotensin II receptor blocker), amlodipine besylate (a calcium channel blocker), and hydrochlorothiazide (a thiazide diuretic). This combination is primarily used as substitution therapy for patients whose blood pressure is already adequately controlled by the simultaneous intake of the three individual components at the same dosage strengths. By targeting the renin-angiotensin-aldosterone system via AT1 receptor blockade, inhibiting calcium-dependent vascular smooth muscle contraction, and regulating fluid volume through diuretic action, the drug provides additive blood pressure-lowering effects. Candesartan cilexetil acts as a prodrug that is converted to its active form, candesartan, during gastrointestinal absorption.
