Overview

Amoxicillin + erythromycin is an antibiotic combination regimen being evaluated by the University of Melbourne as part of the PROMOAT (Preterm Rupture of Membranes Optimising Antibiotics Trial) domain within the PLATIPUS (Platform for Adaptive Trials in Perinatal Units) adaptive platform trial. This combination consists of two small molecule antibiotics: amoxicillin, a beta-lactam that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, and erythromycin, a macrolide that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. The regimen is administered orally for seven days or until delivery to pregnant women with preterm premature rupture of membranes (PPROM) before 37 weeks' gestation. The trial aims to determine the efficacy of this combination in preventing maternal and fetal infections caused by pathogens such as Group B Streptococcus, Ureaplasma species, and Escherichia coli, thereby improving neonatal health outcomes as measured by the PLATIPUS Ordinal Outcome Scale.
