Amphotericin B is a polyene antifungal medication used primarily to treat serious and potentially life-threatening systemic fungal infections and leishmaniasis. It is not indicated for minor fungal infections. The drug acts by binding to ergosterol, a key component of fungal cell membranes, forming pores that disrupt membrane integrity and cause leakage of monovalent ions (Na+, K+, H+, Cl-), leading to cell death. Amphotericin B has a higher affinity for ergosterol than cholesterol but can also bind mammalian cell membranes, contributing to its toxicity profile. It is available in several formulations including conventional (deoxycholate) and lipid-based forms such as liposomal amphotericin B (AmBisome). The drug was originally developed in the 1950s.
